Crossroads Community Care

Mission

Crossroads has a mission to serve young adults and adolescents who are experiencing mental illness, by improving their ability to live independently.

Crossroads will utilize a comprehensive, collaborative, and community services approach that will allow young people with mental illness to remain in the community. Crossroads is dedicated to a client-directed and outcome-informed approach to serving clients.

Crossroads staff will collaborate with clients and families to individualize treatment plans utilizing the following services:

  • Linkage to Psychiatric Services
  • Individual Therapy
  • Family Therapy
  • Targeted Case Management
  • Life Skills Training
  • Parental/Caregiver Support

Services are provided and coordinated by the Crossroads multidisciplinary team which includes a therapist and targeted case manager. Services are individualized and often do not fit into the standards of medical necessity which are critical to achieve positive outcomes. The family is treated as a unit and all family member needs are addressed. Crossroads plans to serve individuals residing in Palm Beach County.
