This 5-minute video depicts a theory of change for achieving breakthrough outcomes for vulnerable children and families. It describes the need to focus on building the capabilities of caregivers and strengthening the communities that together form the environment of relationships essential to children's lifelong learning, health, and behavior.
As Community Partners developed our own theory of change specifically for Palm Beach County, we built a model for community and family development based on expert housing, community and mental health services. Our unique blend of programs addresses a family's whole situation, in the context of their particular needs. Additionally, our program teams target the most toxic and high-risk neighborhoods and communities in Palm Beach County, providing tools that empower families with the knowledge and skills to improve their lives for the long term. With each child that is raised with less trauma, abuse and neglect, the greater odds he or she will reach full and healthy development. With each family that reaches stability, the greater the odds the community will thrive with strength and vitality.
