Community Partners offers two exceptional mental health programming options for children and youth:

  • Target Case Management
  • Crossroads Community Care

About Targeted Case Management:

The Targeted Case Management (TCM) Program works with children and their families in the home, school, and community to develop strength based individualized service plans designed to meet their physical, safety, and social-emotional needs. They work to empower children and families by enhancing access to community resources and ensuring families are able to utilize these resources. Our TCM managers also coordinate access to services, monitor service delivery, advocate for client and family needs, and evaluate service outcomes. The program focuses primarily on the prevention of out-of-home placement, helping clients transition smoothly into less restrictive levels of care as well as the stabilization of the family impacted by mental health issues. When appropriate, our TCM program utilizes the best practice of High-Fidelity Wraparound. Wraparound services focus on the internal strength of the family and deliver unconditional care to children and families with serious mental and behavioral health concerns. This approach is highly individualized, strength based and needs driven.

Any child, adolescent, or family residing in Palm Beach County is eligible for an assessment to receive TCM services.

About Crossroads Community Care:

Crossroads Community Care serves individuals ages 14 – 24 years old who have been diagnosed with a serious a mental illness. The program ensures that these individuals are able to remain in the community through the provision of intensive therapeutic interventions, targeted case management, and psychoeducation. Crossroads works with the client to build the skills necessary to maintain independence in school and/or work. The program also works with the client’s natural support system to understand the long-term symptoms and treatment of the client’s diagnosis.

Individuals ages 14 – 24 years who reside in Palm Beach County are eligible to be assessed for the Crossroads program.
